How is a rear windshield wiper changed?

Why do the wiper blades on my car keep coming off?
A shaft for the wiper system enters the bulkhead; a nut fitted onto that shaft secures it to the bulkhead and prevents it from moving. The wiper will overshoot on both sides if the nut is loose. A button with a setscrew is attached to the shaft and holds the button to the wiper shaft, which protrudes through the bulkhead.
